血糖控制‧生活重點

本身是預防醫學專科的黃澤奇醫生表示,無論是一般人或糖尿病患者,控制血糖的要訣皆一樣,即飲食、運動和藥物,三管齊下。
“飲食是指健康飲食,即少糖、少鹽、少油、少吃加工食物,要多吃蔬果、纖維、穀類及新鮮食物。運動最好是每天30分鐘,而藥物就是依照醫生的指示注射胰島素或口服藥物。"黃澤奇醫生強調,病人需要自律是非常重要的,除了定時自我檢測血糖值,也要注意生活方式和飲食習慣。
他認為,雖然對糖尿病患者來說改變生活方式和飲食習慣很重要,但最重要的還是改變患者的觀念和想法,讓他們瞭解所服用藥物的原因,才能達到糖尿病治療的關鍵成效。
4天自我檢驗血糖值“所謂解鈴還須繫鈴人,不只是糖尿病患者,所有病人都應該為自己的病情而負責任,持著正確的觀念去改變生活方式和飲食習慣,同時加強醒覺性。"他說,藥物並不是解決疾病的根本方法,患者要掌控自己的健康,即使服用了藥物,飲食習慣也須要有規律化,否則就會功虧一簣。
他建議糖尿病患者最少一個星期4天自我進行驗血糖值測試,即分別在早午晚三餐,用餐前和用餐後的兩個小時驗血,那麼就可以更瞭解本身對那一餐的反應,從中做出飲食上的調整。
他指出,空腹時血糖水平須不超過6.1mmol/L;餐後則不可超過8mmol/L。空腹是指8至12個小時沒有進食,4至6mmol/L是屬於正常水平,若少過4mmol/L就屬於過低,年長者會隨時休克暈倒。
黃澤奇醫生透露,大部份的糖尿病患者都曾被親友“挑釁"或“煽動"食用不健康的美食,如果患者的意志力不夠堅定,就會離健康之路越來越遠。
“通常不營養的食物會比較好吃,這對糖尿病患者來說絕對是控制血糖的主要挑戰之一。患者只要意志力堅定,懂得如何控制飲食,就能更易控制血糖,避免併發症纏身。"通常,黃澤奇醫生在替糖尿病患者初診後,都會建議他們見飲食治療師學習如何改善飲食習慣,以控制好血糖值,避免引起併發症。

助病患注意均衡飲食
現今科技發達,很多患者都會上網瀏覽健康資訊,但往往會一頭栽入單一食療,導致攝取營養不均衡。飲食治療師徐美滿強調,均衡飲食很重要,尤其是糖尿病患者更需加以注意。
她舉例道,網上傳吃苦瓜對疾病有所幫助,於是病患就一直狂吃,那是不正確的飲食方式。雖然苦瓜有一定的營養價值,但是也得平衡飲食。
飲食治療師徐美滿建議,患者可使用食物金字塔搭配我的餐盤的方法來用餐,當中以全穀類為首要食物。
食物金字塔是把營養豐富的食品,有系統的歸類,我的餐盤(MyPlate)則是依據個人體質來衡量每餐食物的攝取量、營養是否均衡。
注意隱藏性糖分
她呼吁親友應以同理心來對待糖尿病患者,比如與患者共餐時吃相同的食物,而不是患者吃糙米,親友吃白米。親友應該陪伴患者一起用餐,從中鼓勵患者。
她叮嚀糖尿病患者注意“隱藏性的糖"。“很多美味可口的美食都是不適合糖尿病患者,比如說在外頭販賣的食物,烹飪期間一定添加了不少的糖,即使一杯普通的飲料,當中亦隱藏著不為人知的糖份。"她說,很多糖尿病患者從小就培養出重口味,一時要他們戒口是較為困難,因此可以鼓勵患者以代糖取代白糖,或飲用糖尿病特製飲品。
她分析道,雖說飲用糖尿病特製飲品有助患者減少糖份的攝取以控制血糖,但是若飲用方式錯誤,如用餐後再飲用,就會添加糖份的攝取量,進而引發反效果。飲食治療師徐美滿強調,要把甚麼食物吃進肚子,選擇權在於自己,而不是別人。糖尿病患者應該為自己的健康著想,選擇正確的飲食方式,以控制血糖為主要的方針。
